Health Insurance Plans
Child Health Plus – Children under the age of 19 yrs
Applicants may choose:
- Excellus Blue Cross Blue Shield
- Fidelis Care
No co-pays, possible premium, and dental coverage. Coverage begins on the 1st of the enrolled month.

Medicaid - Adults and Children, Birth to 64 yrs
Applicants may choose from the three managed care plans below:
- Blue Choice Option
- MVP Option
- Fidelis Care
Co-pays may apply in some instances for adults only, no premiums, prescription and dental coverage will be provided through Medicaid. Medicaid coverage will start the 1st of the month of the application. If you have paid or unpaid medical bills from the past 3 months, Medicaid may be able to pay for these costs. Option plans begin 30 – 60 days after Medicaid coverage starts.

Family Health Plus – Adults 19 to 64 yrs

Co-pays apply, no premiums, and prescription coverage will be provided through Medicaid. All other services are covered under the plan chosen. Coverage begins on the 1st of the enrolled month.

Family Health Plus – Premium Assistance Program - Adults 19 to 64 yrs
Available for individuals who are eligible for, or enrolled in, employer-based insurance. You must be income eligible for Family Health Plus. Department of Human Services may pay your share of the insurance premium, if cost-effective, or you will be allowed to enroll in Family Health Plus.
* Medicaid and Family Health Plus applicants may not receive notification regarding coverage for 2 – 3 months after the application has been submitted.
Pregnancy Care
If you are pregnant and live in New York State, you may be eligible to receive complete medical care during your pregnancy and delivery, and for at least two months after delivery.
