Providing Innovative Solutions in Human Services Delivery

CCSI works closely with our customers to provide the infrastructure essential to effective, efficient project management.

Project management services include:

  • Project management staffing;
  • Information technology and data analysis;
  • Financial management, including budget development, monitoring, and funds management; and
  • Facilities management, including providing appropriate office space

One example of our work in this area is the Western NY Care Coordination Program, an innovative, collaborative effort to transform the way services are provided to individuals with serious mental illness—increasing engagement by addressing the specific needs and wants of the individual, and utilizing wraparound funding to support individual recovery goals. Central to the success of this initiative has been the ability to engage consumers and family members in the program’s design, implementation, and ongoing evaluation.

Performance data point to a number of important improvements in the quality of life among people receiving services under the WNYCCP, including:

  • A reduction in the use of acute mental health services—hospital days and emergency room visits;
  • A decrease in reports of self-harm or physical harm to others; and
  • An increase in the number of people engaged in gainful work-related activities

The Facilitated Enrollment Program provides another example of CCSI’s work in the area of program management. Since 2003, CCSI has served as Lead Agency for the New York State Department of Health-funded Facilitated Enrollment (FE) program for Monroe and Livingston counties. The goal of this program is to ensure all uninsured children, teens and eligible adults have access to affordable health insurance. Facilitated enrollers work with individuals and families to determine if they qualify for various types of publicly supported health insurance, including: Child Health Plus, Family Health Plus, and Medicaid—and assist them in completing all stages of the application process. As lead agency for this important initiative, CCSI contracts with five community-based providers to provide outreach and enrollment services from 12 community locations.

Our Staff

As Director of the Western New York Care Coordination Program (WNYCCP), Adele Gorges is responsible for overseeing an extremely complex set of programmatic, clinical, regulatory, fiscal, and technical initiatives designed to improve services to persons diagnosed with serious mental illness. In addition to her strong project management capabilities, Adele brings to the project significant experience in the behavioral health finance field. She has presented on the WNYCCP in a number of forums, including the American Psychiatric Association and the New York State Association for Psychiatric Rehabilitation Services (NYAPRS). Prior to joining CCSI, Adele served as Director of Finance for Rochester General Behavioral Health System. She holds a degree in Political Science from Manhattanville College.

As Program Manager, Kim Wynn oversees all aspects of the Facilitated Enrollment initiative, including working closely with the team of local provider agencies and community-based enrollers. Kim brings to this project more than ten years of experience in the human services field. Prior to joining CCSI, she was an Examiner with the Monroe County Department of Human Services. Kim holds a degree in Business Administration from Dillard University.
