We provide long-term solutions to the problems of homelessness, addiction, incarceration, and poverty by helping the people we serve overcome obstacles and develop healthier, more self-sufficient lives. Case Manager – Residential Reentry Center Rochester, NY Volunteers of America – Upstate New York is seeking an energetic human services professional to join our Residential Reentry Center team. If you’re passionate about advocating for low-income and vulnerable people, delivering effective social services, connecting with community partners and reducing recidivism, we want to hear from you! JOB DUTIES AND RESPONSIBILITIES •Provide all aspects of Case Management as defined by VOA and the Federal Bureau of Prisons for a case load of individuals living in a Residential Reentry Center or on Home Confinement. • Establish good working relationships with related agencies to maintain a comprehensive referral network for residents. •Conduct home visits and employment checks in accordance with VOA policies and relevant regulatory and funding sources. •Provide pro-social life skills training on an individual and group basis, and provide follow up to assure residents are achieving set goals. •Transport and coordinate transportation for residents to services when needed. • Maintain accurate records for all residents in accordance with VOA policies and relevant regulatory and funding sources. •Maintain open communication, positive relationships and referral assistance for residents as needed. •Promote positive attitude and model case management expectations for other staff. •Coordinate caseload coverage when other case managers are out of the office. •Assist with staff file audits. •Other duties as assigned. EXPERIENCE AND EDUCATION QUALIFICATIONS •Associate’s Degree in Human Services, Social Work or related field. Bachelor’s preferred. •Valid NYS Driver’s License •Availability to work on-call and occasional evenings and weekends as required •Strong organizational skills •Working knowledge of MS Office products •Ability to pass a Bureau of Prisons criminal background screening •Ability to pass a drug screen HOW TO APPLY: Qualified candidates should apply at www.voaupny.org/jobs or mgourdine@voaupny.org.
