Crisis to Stability
The Crisis to Stability (C2S) program works with individuals who recently experienced homelessness and are transitioning into permanent housing, or are at risk of losing their current housing. This 9-month program utilizes Critical Time Intervention and Peer Support to ensure that participants are connected to longer-term supports and services needed to successfully maintain housing. C2S is grant-funded, non-billable, and can work in conjunction with any other services or programs.
Eligibility Criteria
The individual must have a diagnosis of a Severe Mental Illness or Co-Occurring MH/SU Disorder and fall into one of the following categories:
- Experiencing homelessness but has a housing plan
- Has current housing but is at imminent risk of homelessness
- Currently being provided specialized housing services but requires more intensive support to maintain placement
Critical Time Intervention
- Community- and evidence-based case management model
- Provides intensive, time-limited (9 months) support to help people through a critical transition in their lives
- Focuses on housing stability/preventing homelessness
- Strengthens long-term supports
Peer Support
- Peer Support Specialists have been successful in the recovery process and help others experiencing similar situations
- Helps people stay engaged in the recovery process through shared understanding, respect, and mutual empowerment
- Reduces the likelihood of relapse or re-occurrence
