Bilingual Community Health Worker

The Community Health Worker (CHW) provides perinatal education, support, referrals, and care coordination during home visits for a caseload of high risk, vulnerable pregnant and parenting women with a goal of reducing barriers to care and achieving a healthy birth and postpartum outcome. The CHW works to establish and maintain trust, provides needed support and advocacy for Mothers and their families, and participates in regular review of program outcomes. This position is full-time (40 hours/week) and is exempt.
