The Case Manager will be responsible for helping residents, with special needs and/or mobility and hearing/visual impairments access community resources and services. The Case Manager will work with residents to determine what services may be necessary and then helping them setup appointments and secure transportation if necessary.
ESSENTIAL JOB RESPONSIBILTIES:
• Meet with residents with 48 hours of arrival complete an assessment.
• Assessment and referral as needed to legal and financial assistance programs.
• Assessment and referral as needed to meals/nutrition programs.
• Assessment and referral as needed to counseling programs.
• Personal care and other home care assistance to meet identified needs.
• Service Coordination of and/or facilitation of other medical, social and support services that may be needed by individual residents.
• Complete case notes on residents regarding the services they are receiving.
• Assure that all regulatory approvals are met.
• Assure residents are provided transportation as needed.
• Establish and maintain good working relationships with related agencies to ensure Volunteers of America of Western New York, Inc. maintains a comprehensive referral network for residents.
EDUCATION & QUALIFICATION REQUIREMENTS:
• Bachelor’s degree in Social or Behavioral Science program from an accredited college or university.
• 3 year experience working in Social Services field in a relevant position
• Combination of education and experience may be considered.
• Must obtain First Aid & CPR certification with ninety days of hire.
• Proficient with computers and Microsoft Office
• Valid NYS Driver’s License
• Availability to work a minimum evenings and weekends as needed; flexible hours as required to me resident needs.
