As a Children’s Health Home Care Manager, your superior coordination ability, attention to detail and follow through allows for exceptional case management for children who need it most.
STATUS: Full-Time (40 Hours)
LOCATION: Monroe County/Hybrid
DEPARTMENT: Children’s Health Home Care Management
SCHEDULE: Monday – Friday/ Day Shift
ATTRIBUTES
- Bachelor Degree required
- Background working with children, parents and families
- Valid NYS driver license – must be willing to go out within the community
- Strong analytical, computer and communication skills
- Excellent organization skills
RESPONSIBILITIES
- Case Coordination. Create, manage and coordinate across all continuums of the enrollee’s care plan, including medical/behavioral and social service needs and goals
- Community Outreach. Effectively perform progressive community outreach with assigned case and move to enrolled status as appropriate
Documentation. Complete documentation within established timeframes
PHYSICAL REQUIREMENTS S – Sedentary Work – Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
LICENSES/CERTIFICATIONS QHP – Qualified Health Professional – NYS Office of Alcoholism and Substance Abuse Services (OASAS); NYS DL – valid New York State Driver’s License – New York State Department of Motor Vehicles (NYSDMV)
