| The HUB Training Coordinator provides training and technical assistance in support of Care Management Agencies and Care Managers working within the BestSelf and Huther Doyle Health Homes supported by HHUNY. Training programs developed and implemented will support delivery of person-centered/family driven-youth guided, recovery-focused care management in a manner consistent with New York State requirements, HHUNY policies/practices/procedures, and Netsmart CareManager as the Health Information Technology (HIT) tool. This position provides in-person and web-based trainings, as well as coaching, mentoring and team-based learning to sustain change in practice and associated improvement in care management outcomes. This position additionally provides training for community-based organizations and conferences. While this position is fully remote, it requires the employee to be located in one of the following counties: Monroe, Ontario, Erie, Wyoming, Genesee, Orleans, Wayne, Livingston or Niagara. Travel to CMAs will be required occasionally when needed. |
