SUMMARY
Under the general direction of the Waiver Program Manager the Individualized Care Coordinator (ICC) provides care coordination to seriously disturbed youth (SED) and their families including referring and overseeing the delivery of the remaining two services to youth and families in the Waiver Program; Carries out all job responsibilities with fidelity and accountability to the Agency’s Mission, Vision, Values, Guiding Principles and Strategic Plan.
MINIMUM QUALIFICATIONS:
EDUCATION: Masters Degree or Bachelors Degree in varied disciplines related to Mental Health, Medical, Education or Human Services
EXPERIENCE: Masters Degree and two years experience or Bachelors Degree and four years experience required.
SKILLS: Working knowledge of the CFT and Care Coordination process.
SPECIAL REQUIREMENTS: Valid NYS Driver’s License
