General Description
Under general supervision, coordinates the intake process and ensures that State and Agency regulations are met.

Essential Duties and Responsibilities
Provides initial screening and enrollment of clients referred for program services.
Provides information to agencies and clients about program services assuring clients are enrolled in appropriate services.
Completes an identity verification process for incoming clients in accordance with department protocol.
Initiates all necessary paperwork and intake packet materials in compliance with Agency and regulatory standards.
Maintains efficient communication with intake team and Department Director.
Evaluates client needs and assists clients as necessary.
Refers clients to appropriate community services or case management advocating on the client’s behalf when appropriate.
Assists team members with requests for additional information from referral sources.
Provides crisis intervention and conflict management to residents as necessary.
Maintains current knowledge regarding client needs and community resources.
May assist clients in obtaining a Primary Care Physician, selecting an HMO, and completing Medicaid verification process.
Maintains all paperwork, files and rosters for the program.
Enters and updates demographic and insurance data for new clients enrolling or participating in agency programs into the appropriate system(s).
Maintains files for the department and files new client paperwork.
Performs billing duties as requested by supervisor.
Maintains standards and performance metrics.
Verifies the presence of all reported clients and addresses any changes daily.
Compiles necessary information to generate reports regarding client progress, program status and quality assurance as needed.
Participates in relevant agency meetings and ongoing development of professional knowledge and skill.
Attends supervision as indicated, program and departmental meetings, and CFC quarterly meetings.
Develops client service and clinical intervention skills through involvement in agency in-service training, conference attendance, and supervision sessions.
Is aware of and supports the Agency’s cultural competence goals, efforts and activities.
Acts as a backup coverage as necessary.
Adheres to all applicable federal and state laws, including but not limited to those governing client confidentiality, privacy, billing and documentation.
