Summary: The Licensed Practical Nurse is responsible for performing duties outlined in the plan of care while providing ongoing support, education, and empowerment to patients and their families.
Duties include:
- Providing personal care and emotional support to patients and families
- Providing anticipatory guidance to patients and families throughout the hospice care and palliative care
- POS Documentation
- Report changes to RN Case Manager, Supervisor, Team Leader and/or On-Call Nurse and complete notification to team within electronic medical record
- Recognize and report situations of potential harm to RN Case Manager, Supervisor, Team Leader, and/or On-Call Nurse.
- Work with an collaborate with all members of the interdisciplinary team
- Participate in orientation, onboarding, and mentorship of new team members
- Provide informational visits, admissions, and/or frequency visits as assigned
Qualified candidates will possess a valid and current NYS License Practical Nurse license and driver’s license. Experience in nursing and home care preferred, but not required.
