III. GENERAL DUTIES:
The Open Access Evaluator will provide direct service to clients, parents/families, and professionals. The Open Access Evaluator will complete a psychosocial assessment on individuals presenting for services and determine the appropriate level of care, using the OASAS approved LOCADTR. The Open Access Evaluator will be responsible for referring and linking the client to the appropriate services. This is a full time position.
IV. JOB SEGMENTS:
a. Complete psychosocial assessments in accordance to OASAS regulations and Agency policy.
b. Complete a level of care determination utilizing the LOCADTR.
c. Refer and link clients to the appropriate level of care.
d. Provide individual counseling to clients and their family members.
e. Complete all required paperwork within OASAS and Agency timeframe.
f. Provide data and statistics to the Program Coordinator.
g. Knowledge of treatment options in the Finger Lakes Region.
h. Ability to work independently.
i. Other duties as assigned.
V. Qualifications: The minimum requirements for this position are:
a. Qualified Health Professional (CASAC, MSW, LMHC, RN).
b. Experience working with clients with a Substance Use Disorder (SUD).
c. Experience conducting evaluations and completing psychosocial assessments.
d. Experience using the LOCADTR.
e. Knowledge of OASAS regulations.
