Position: Physician’s Assistant or Nurse Practitioner
Status: Per Diem, 8-15 Hours Per Week
Pay Rate: $65/Hour (statutory benefits only)
Qualifications:
•Nurse Practitioner or Physician Assistant (PA) from an accredited program
•3+ years of experience working in addiction treatment, either in a primary care or specialty practice
•American Society of Addiction Medicine Certification (ASAM) certified preferred
•DATA-2000 Waiver required, preferably with 100-patient limit. 30-patient waiver acceptable.
•Must have your own DEA/DEAX license without restrictions
•CPR and First Aid certifications (BLS or ACLS certification preferred)
•Experience with Motivation Interviewing techniques is preferred
•Prior experience in Primary Care or Psychiatry a plus
Job Duties:

• Perform initial and on-going assessment of patients’ medical, physical and psychosocial
status
• Obtain and evaluate health and medical history; perform a physical exam, conduct preventive
screening procedures based on age and history
• Identify medical and health risks
• Complete accurate and timely documentation of observations, interventions, plan of care,
referrals and follow-up
• Formulate appropriate differential diagnosis based on history, physical examination and
diagnostic testing
• Make appropriate referrals to other health professionals and community agencies
• Reassess and modify plans as necessary to achieve medical and health goals
• Contribute continuing education, expert blogs, and ongoing outreach marketing activities
• Practicing under physician supervision: team-based model of treatment.
• Chart reviews and feedback
• Close coordination of care with CASAC team
• Motivated to stay current with best-practices and new research
• Other duties assigned.
