This position is responsible for the delivery of social work services to identified home care clients. The Social Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed.
STATUS: Full-time (40Hours)
LOCATION: Monroe Ave – Rochester, NY
DEPARTMENT: Skilled Nursing Facility Hospice – Social Work
SCHEDULE: Monday – Friday/ Day Shift
ATTRIBUTES
- Bachelor’s Degree in health related field with a minimum two years related experience; Bachelor/Master in Social Work preferred
- Home visiting experience strongly preferred.
- Proven psycho social assessment skills.
- Effective verbal and written communication skills essential.
- Ability to stay organized and flexible important.
- Possession of a valid NYS driver’s license and own car required.
RESPONSIBILITIES
- Patient Care & Service. Interprets to the home health team the social factors in the patient situation that may have bearing on his/her progress.
- Planning. Determines the need for continued social work service and draws upon other community agencies that can further assist the patient and family.
- Communication. Serves as liaison with social work departments and other health and social agencies in the community.
- Compliance. Maintains high regard for patient privacy in accordance with the corporate privacy policies and procedures.
PHYSICAL REQUIREMENTS S – Sedentary Work – Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
