Summary: The Nurse will assist with coverage needs for a minimum of 10 hours per month to provide adequate support to patients, families, and the CareFirstNY team on a flex basis. Must be able to work flexible hours and ability to update availability on calendar weekly.
Duties will include completing assessments, frequency visits, admissions, and death visits as assigned by the Team Managers. Nurse will coordinate patient care with the full time Case Managers for a caseload of patients who are terminally ill and will communicate/educate in a manner to advocate, empower, and complete nursing in a fiscally responsible manner.
Our current openings include:
- Nurse (Flex): 8 AM – 4:30 PM Availability| Must be available for a minimum of 10 hours per month.

Skills/Qualifications:
Must have a valid and current NY State Professional Registered Nurse’s License and valid driver’s license.
